How to Reduce Patient No-Shows: Risk Scores, Deposits and a Smart Waitlist
An empty appointment slot costs the clinic the same as a busy one. Here is a practical system to find the visits most likely to be missed, protect them, and refill the ones that fall through.
Why Patients Miss Appointments
Most no-shows are not rude patients. They forgot, the visit was booked weeks ago, traffic was bad, or the problem went away. Some are predictable: a patient who missed twice before, a booking made a month ahead, a first visit with no deposit.
Step 1: Remind at the Right Time
A reminder the day before, with a one-tap way to confirm or cancel, does more than a call on the morning of the visit. A cancellation you hear about early is a slot you can refill.
Step 2: Score the Risk
Give each booking a risk score from signals you already have, and show staff the reasons so the score is never a mystery:
- The patient's past no-show rate
- Days between booking and visit
- First visit or returning patient
- How the booking was made
- Time of day
- Any unpaid balance
Start with simple points. After a few months of real outcomes, a model trained on your own clinic's data can replace the points.
Step 3: Protect High-Risk Visits
- Ask the patient for an extra confirmation.
- Optionally require a small deposit, fixed or a percentage of the service.
- Count the deposit as a payment when the visit is billed, so nothing is charged twice.
- Keep the deposit for late cancellations and refund it for early ones, with a rule patients can read.
Step 4: Refill Freed Slots from a Waitlist
Patients who want an earlier appointment join a waitlist with the earliest and latest time they can come. When a visit is cancelled, offer the slot to a few matching patients at once. The first to accept gets it, and unanswered offers expire quickly so the next person is asked.
Clinics Manager scores every booking, shows the top reasons, supports optional deposits that convert into invoice payments, and offers freed slots to up to three matching waitlist patients.
Measure What Changed
Track the no-show rate by risk band, by doctor and by booking source, before and after you change the policy. If deposits help but hurt bookings, adjust the threshold rather than dropping the idea.

- Will deposits scare patients away?
- Apply them only to high-risk bookings or expensive services, and explain the refund rule clearly. Most patients accept a deposit when the rule is fair.
- Can a risk score be unfair to a patient?
- It should only trigger a confirmation or a deposit request, never a refusal of care. Showing staff the reasons helps them judge each case.
- How are deposits paid?
- In Clinics Manager the deposit is paid at the desk or by transfer and recorded by staff. Online payment gateways are not part of it yet.
